Before You Arrive

• Insurance card: We are currently in-network with Delta Dental, and we also work with patients who have other insurance plans that offer out-of-network benefits.
• Health history: A complete list of current medications, allergies, and any recent diagnoses will help Dr. Patel provide safer, more personalized care.
