Before You Arrive
A little preparation goes a long way toward making your first appointment smooth. We ask new patients to complete intake paperwork in advance, which covers your health history, current medications, and insurance information.
Here are a few things to have ready before your appointment:
- Insurance card: We are currently in-network with Delta Dental, and we also work with patients who have other insurance plans that offer out-of-network benefits.
- Health history: A complete list of current medications, allergies, and any recent diagnoses will help Dr. Patel provide safer, more personalized care.
- Questions: We encourage you to write down anything you have been wondering about, whether it is a tooth that has been bothering you or a cosmetic concern you have been putting off.

If we find signs of gum disease during your exam, we may recommend a different type of cleaning called scaling and root planing, and we will explain that recommendation in detail if it applies to you.
